JOB SUMMARY

Working independently and collaboratively in a team-based care model, the Registered Nurse (RN) is responsible for direct patient care at Rock Creek Medical and partners with, and complements members of the primary care team in providing holistic, person-centred, and culturally-appropriate care. Specific tasks include a focus on providing assessment, case management, screening, healthy lifestyle support, education, and chronic disease management with a goal of improving health outcomes and facilitating access to services for the entire patient population. The RN will initiate, implement, and monitor health care plans in collaboration with the patient, practitioners and other members of the interdisciplinary team.

JOB SUMMARY

The RN will initiate, implement, and monitor health care plans in collaboration with the patient, practitioners and other members of the interdisciplinary team. 

REGISTERED NURSE ROLE

The RN will provide a range of services, including those outlined below, and other duties as assigned from time to time.  

TYPICAL DUTIES AND RESPONSIBILITIES

  1. Delivers quality nursing patient care by: 
  • Performing nursing activities, including crisis intervention, emergency assessment and care, point of care testing, specimen acquisition, in accordance with established policies, protocols and procedures, and arranging referral where necessary.
  • Assessing, diagnosing, planning, implementing, documenting, and evaluating the care needs and goals for individual patients, caregivers/families, and communities. 
  • Collaborating with staff as required, including patient specific care instructions.
  • Task management including lab requisitions, colon screening follow-up, cervical cancer screening reports, BCCA notices, INRs, TSH, diagnostic requisitions/follow up and any specific tasks sent from practitioners that require follow up.
  • Attends patient visits independently for ear syringing, ROS, self-injection teaching (IM and SC), dressing changes, UTIs, MoCA and mini-mental exams, and Drivers Medical eye exams, cervical smear (PAP), simple biopsies, reassessments
  • Checking results for practitioners during absences, assessing the urgency and escalating according to clinic protocols.
  • Utilizing a variety of established health education, promotion, and service interventions, such as the Well Child Program, and Pre/Post-natal Care.
  • Optimizing capacities for health by modeling, counseling, and teaching life skills for individual patients and their caregivers/families.
  • Conducting primarily in-person patient care, with a small amount of virtual and home visits as applicable
  • Collaborating with the multidisciplinary team of health care professionals and health care services and providers to coordinate and facilitate quality patient care.
  1. Provides health education and promotion and treatment services for patients, including individuals, families, care teams, groups, and communities. Areas of focus include high cholesterol, diabetes, proper puffer usage, hypertension, CHF, and weight loss. 
  2. Documents and reports nursing care, in accordance with policies, protocols, procedures, and standards. Protects and secures all patient records and supplementary related documents by monitoring access and maintaining appropriate disposition of the documents/information to protect confidentiality. Provides patient data to specified healthcare professionals.
  3. Participates in case conferences, treatment plan reviews, and staff meetings.
  4. Promotes good public relations and community awareness within the multidisciplinary team, and with others, including community resources.
  5. Contributes to a safe and healthy working environment by observing and promoting universal precautions and infection control procedures; removing obvious hazards; reporting faulty equipment, accidents, injuries and near misses; and participating in quality improvement.
  6. Ensures competency and professional practice standards are met, in line with the regulatory controls on practice.
  7. Accessing education, continuing professional development, and training as required to maintain and expand competence to meet patient needs.
  8. Participates in fire prevention and disaster plans as required.
  9. Performs other nursing-related duties as required.

QUALIFICATIONS

Education, Training, and Experience

  • Registration with BC College of Nurses and Midwives as a practicing RN registrant in good standing.
  • Preferably five (5) years of recent related primary care or community experience, knowledge of available programs and services including their interrelationships, and their function in delivering care in the community; or an equivalent combination of education, training, and experience.
  • Basic Life Support, CPR Level C certificate.  
  • Valid BC driver’s license may be required.

The ideal candidate is an experienced and compassionate nurse who is reliable, takes initiative and has the following knowledge and capabilities:

  • Certifications in Sexual Health (Contraception/STI testing/cervical cancer screening) and/or Diabetes education considered an asset
  • Comfort in chronic disease management considered an asset
  • Experience with primary care nursing considered an asset

  • Apply nursing process as required and described in BC College of Nurses and Midwives Standards of Nursing Practice.

  • Foster and promote good public relations.
  • Physically and emotionally able to carry out the duties of the position.
  • Use of a personal vehicle may be required.

  • Operate equipment, including computer and utilize the Electronic Medical Record software (Profile)
